Are employees entitled to a copy of all documents in their personnel file? This includes notes taken during and disciplinarian meetings.

That would depend on where you live - laws vary.
But in most places, no.

In the united kingdom, yes you are allowed and legally able to see your personal file, all notes from meetings also should be provided to you within I think three days. Xx
